New Patient Registration

To be eligible to register at the practice, you must live within the practice catchment area.

Currently our boundary encompasses Tonbridge (north of the river Medway), Hildenborough, Leigh, Weald, the Tonbridge side of Hadlow, Underriver, Shipbourne, Chiddingstone Causeway and homes within this area (see map below).

Register using the link below. Please ensure this is done before making an appointment, unless it is an emergency. You will then be added onto our computer system and we would suggest leaving at least one week before you make an appointment. This will enable the medical team to use the computer system when they see you, and potentially have access to the electronic notes from your previous surgery.

As it often takes some time for paper records to be forwarded from your former practice all new patients are asked to complete a health questionnaire.  Medical treatment is available from the date of registration. Please contact our Care Navigators for further information.

Pre-Registration

If you wish to register with our practice, you should complete the form as above.

Once registered you will be able to make appointments – please allow at least one week.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Your Named GP

The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.

We know that some patients will wish their named GP to be a particular doctor. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice as you currently do. Your named GP will not be available at all times.

If you wish to be told the name of your accountable GP, please ask the Care Navigators when you are next in the surgery.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can be seen as a temporary patient.

You can register as a temporary resident if you plan to live near the GP surgery for up to three months. This will allow you to be on the local Practice list but still remain a patient of your permanent GP. After three months you will have to apply to register with that surgery as a permanent resident.

You only need to contact us when you require treatment, you do not need to pre register. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

We acknowledge it has been very frustrating for patients waiting in phone queues to get an appointment. However, GP capacity is struggling to meet the demand.

Online access

In some cases this is not possible or patients wish to have further access (to view medical records and clinical documents). To set this up you are now required to complete a short form (Online Access Form) at the surgery. Please bring along with you with two forms of ID. A Care Navigator will then verify your identity and pass the form on to the admin team to process. Once processed you will receive an email from ‘Your Healthcare Provider” with an OS code and linkage key, to complete your set up.

Proxy Access

You may wish to access another person’s online account to order medication, book appointments ect on their behalf.

To request proxy access to another person’s online account a Proxy Access form must be completed at the surgery by both parties. Under 11s need not attend but anyone over the age of 11 is required to attend in person with their ID and completed form. For children under the age of 11 a birth certificate would be adequate and no further ID is necessary for them. Those that hold Lasting Power of Attorney do not need to bring along the patient giving the access but must supply a copy of the LPA, if we do not already hold it on record.

Moving to the UK from another country

If you are moving to the UK from another country, please bring your medical records with you. This helps ensure that your new healthcare providers have the information they need to support your ongoing care.

Try to gather copies of your medical records from your current healthcare team—this might include your GP, any specialists you have seen, and hospitals or clinics where you've received treatment. It would be very helpful to have your records translated into English.

It can also be very helpful to bring a brief summary of your medical history. This might include any current health conditions, medications you are taking, allergies, and any major treatments you have had.
Taking these steps can make settling into the UK healthcare system much smoother and safer for you.

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality doesn’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

The NHS App

Hildenborough & Tonbridge Medical Group would like to invite you to use The NHS App

Owned and run by the NHS, the NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet.

To request further access via the NHS app (to obtain things such as test results) you are required to complete a short form and bring it along to the surgery with two forms of ID. This will then be passed on to the admin team to process and the set up codes will be emailed to you.
