Care Navigator
Hildenborough and Tonbridge Medical Group (HTMG) is recruiting a part-time Care Navigator to join our friendly, established team across both our surgery sites in Hildenborough and North Tonbridge.
We are a busy double site dispensing and teaching GP practice serving approximately 17,000 patients. Our Care Navigators play a vital role in helping patients access the right care, and this is a varied and rewarding role for someone who enjoys working with people.
The Role
Working 27.5 hours per week, you will be the welcoming first point of contact for our patients — in person, by telephone, and online. You will work as part of a well-structured team led by a dedicated Care Navigation Lead and Manager, so you will always have clear guidance and support.
We are looking for someone who is:
- Warm, professional, and patient-focused
- Calm under pressure with excellent communication skills
- Flexible and reliable, including availability to cover holiday and sickness
- Organised with good attention to detail
- Experienced in a customer-facing or administrative role (GP surgery experience desirable but not essential — full training will be provided)

Hours
- Monday: 7.45am to 1.15pm
- Tuesday: 1pm to 6.30pm
- Wednesday: 1pm to 6.30pm
- Thursday: 1pm to 6.30pm
- Friday: 7.45am to 1.15pm
Flexibility to cover holiday and sickness is essential.
