Dispensers

Gemma, Sian, Anita and Lorraine are our dispensers and are based at Hildenborough Medical Centre, they will be happy to answer any questions relating to the supply of your medicines.

Pharmacists

Sarah and Preeti

Sarah and Preeti are Clinical Pharmacists and have joined HTMG to support our patients enabling them to benefit from the best use of their medicines and also to identify medicines related issues. They hold clinics for medication reviews. Sarah also holds a diabetic clinic.

Cancer Care Co-Ordinator

Jo is employed by the PCN and based at Hildenborough Medical Centre, Jo supports patients on their cancer pathway with safety netting, ensuring patients also have access to any needed relevant information.

Supporting the practice to ensure patients sent on a fast-track cancer referral pathway, are contacted by the hospital’s clinics, within the relevant time frames, Jo then contacts all patients with a cancer within three months of diagnosis, supporting them and their loved ones through what can be a difficult time.

Jo is part of a greater team supporting eligible patients to access their cancer screenings, whilst also updating quality improvement data in the surgeries screening processes (Bowel/Breast/Cervical).

Whilst there is no screening yet for prostate cancer, Jo is championing those patients diagnosed and also sending reminders to patients referencing their PSA test needs.

Jo has attended many MacMillan Holistic training events, which enables her to actively support our HTMG patients. Signposting is a big part of the pathway for our patients and Jo is also able to offer patients telephone appointments, giving patients the time to discuss their own personal situation.

"We are a small team who specialize in frailty, visiting housebound elderly patients who require acute urgent medical care or who may need a full comprehensive geriatric assessment to evaluate their needs. We work with the GP’s and the wider community team to avoid hospital admission, where appropriate, in this vulnerable group patient group. We work with the patient to tailor care to their individual needs and wishes."
