The team consists of Gemma (Dispensing Lead), Anita (Dispenser) , Lorraine (Assistant Dispenser) and Tallulah (Assistant Dispenser), they are based at Hildenborough Medical Centre, they will be happy to answer any questions relating to the supply of your medicines. Sarah and Brian are Clinical Pharmacists and have joined HTMG to support our patients enabling them to benefit from the best use of their medicines and also to identify medicines related issues. They hold clinics for medication reviews.

"We are a small team who specialize in frailty, visiting housebound elderly patients who require acute urgent medical care or who may need a full comprehensive geriatric assessment to evaluate their needs. We work with the GP’s and the wider community team to avoid hospital admission, where appropriate, in this vulnerable group patient group. We work with the patient to tailor care to their individual needs and wishes."
