my mHealth platform
More than 30,000 people across Kent and Medway are now registered on the my mHealth platform, with users reporting positive outcomes.
The platform is designed to empower patients and support self-management of long-term conditions. It offers access to personalised education, rehabilitation tools, medication reminders and instructions, symptom tracking, and much more—via both app and browser.
Following the extension of the current contract, there is an even greater opportunity to make use of the platform to support people living with long-term conditions, including:
- chronic obstructive pulmonary disease (COPD)
- asthma (ages 13 plus)
- diabetes
- cardiovascular disease.
A self-registration form is available and can be shared with patients to allow independent sign-up.
