We are a large practice (serving a population of over 16,000 patients) in Hildenborough, Tonbridge and the surrounding villages. Our main Medical Centres are located in Hildenborough and at Trenchwood in Tonbridge, with branch surgeries in Weald and Leigh.  Our aim is to provide, by an effective team approach, the highest quality of progressive family health care, in a happy and compassionate environment.

NHS England Patient List Maintenance Project

NHS England have written to patients who had not attended the practice recently asking them to verify their registration.  If you receive this letter it is vital that you respond to stay on HTMG's patient list.

Records, Information and Notification

Following a recent variation to our General Medical Services contract, HTMG is obliged to provide the following services to their patients by 31st March 2015:

Summary Care Record (available by 31st March 2015)

When there is a change / update to the information included in a patient’s medical record, a summary of this information is uploaded automatically on a daily basis to the Summary Care Record using the approved systems provided. If you have written in and asked that your data is not included in the upload then your medical record will have been coded accordingly. See below for further information about the SCR.

Electronic transfer of patient records (already in use)

When a new patients registers with HTMG, the surgery will use the GP2GP electronic transfer system to request a copy of the patients notes. Likewise, if a patient leaves our practice we will use GP2GP to transfer information electronically to the patient’s new surgery. The information is transferred in a safe and effective manner and includes any primary or secondary care information which is in the patient’s medical records.

Patient online services

HTMG offers the following online services to its registered patients:

  • The ability to book, view, amend, cancel and print GP appointments online. (available now)
  • The ability to order repeat prescriptions for drugs, medicines or appliances online. This is available now via this website; however by 31st March 2015 this service will link directly with your patient record.
  • The ability to access and view online any summary information derived from the patient’s medical records and any other data which the surgery has agreed that the patient may access and enable the patient to electronically export or print that information. (available by 31st March 2015)

Summary Care Record

Summary Care Record (SCR) is an electronic record of important information about your health.  It will contain information about the medicines you take, your allergies and any bad reactions you have had to medicines. HTMG is planning to upload your Summary Care Record within the first three months of 2015 so that it is available to other health professionals.  You will be able to access your own Summary Care Record via the HTMG website in the same way that you can book online appointments. For futher information click here: Summary Care Record

Shingles Vaccination

This year the shingles vaccination is being targeted only for patients who were aged 70, 78 or 79 years old as of 1st September 2014 and will be suppled to the practice in batches over the year.  Eligible patients will receive an invitation to attend. 

Online Services

Hildenborough & Tonbridge Medical Group offers the facility for patients to book doctors appointments and order repeat medication online using Vision Online Services (VOS). If you would like to manage your appointments or repeat prescriptions online, you will need to register for the service. Please download the form below (or collect it from the surgery) and give the completed form to one of our reception team so that your registration can be processed.  Please note that two forms of identification will be needed, one of which must be photo ID (eg passport, photo driving licence).  The second form of ID should be a bank statement or similar (but not a utility bill).  A letter will then be sent to you giving details of the steps to follow in order to create your online account.  Once registered you can access the services via the links at the top right of this page.

Protected Learning Time

The practice will be closed for up to 6 afternoons each year to allow for in-house or locally-arranged training events. These dates have been arranged for all West Kent practices by the local Clinical Commissioning Group and IC24 to ensure that primary care teams are getting the essential training and development that they need to deliver the best possible service.  Cover is provided by the out of hours service during this time.
