Most immunisations are free to registered patients and we can provide Malaria tablets, Medical Packs, Blood tests and Certificates.
Newly Registered Patients
All new patients are asked to complete a confidential questionnaire to provide us with a summary of your medical history as it could take us several weeks to obtain your medical records from your previous doctor. Please complete the questionnaire as fully and accurately as possible.
Patients Aged 16 to 75
We strongly support the trend towards health education and screening. If you have not attended surgery in the last three years, we would be happy to see you to perform a brief health check and give you any help and advice that you require, especially with respect to diet, exercise and other aspects of healthy living.
Patients Aged Over 75
It is the practice policy to see all patients over the age of 75 at least once a year, in order to perform a brief health check. This can be done at the surgery by your doctor or a practice nurse or, if applicable, at your home by the district nurse.
